CTPL Claim Requirements

Bodily Injury
1. Original Police Report of the accident.
2. Photocopy of Insurance Policy with Official Receipt (insured vehicle) and Endorsement, if any.
3. Photocopy of Driver’s License.
4. Photocopy of Registration Certificate of the insured vehicle with Official Receipt.
5. Stencils of motor and chassis of the insured vehicle.
6. Original Medical Certificate, Hospital Statement Of Accounts and Official Receipt, prescription with official receipts.
7. Picture of the incident / victim.
8. Original PSA Birth Certificate (if minor).
9. Original duly notarized Compromised Agreement (specify the amount) between the Assured and victim.
10. Original duly notarized Affidavit of Desistance.
11. Photocopy of Valid Identification(ID) Card of the desisting party with three(3) specimen signature.
12. Telephone / Cellphone number(s) of the claimant (Assured & Third Party).

Death
1. Original Police Report of the accident.
2. Photocopy of Insurance Policy with Official Receipt (insured vehicle) and Endorsement, if any.
3. Photocopy of Driver’s License.
4. Photocopy of Registration Certificate of the insured vehicle with Official Receipt.
5. Stencils of motor and chassis of the insured vehicle.
6. Original Medical Certificate, Hospital Statement Of Accounts and Official Receipt, prescription with official receipts (if any).
7. Picture of the incident / victim.
8. Original Burial Receipt.
9. Original PSA Death Certificate.
10. Original PSA Marriage Certificate (if married).
11. Original PSA Birth Certificate (if minor).
12. Original duly notarized Compromised Agreement (specify the amount) between the Assured and desisting party.
13. Original duly notarized Affidavit of Desistance of the desisting party.
14. Photocopy of Valid Identification (ID) card of the desisting party with three(3) specimen signatures.
15. Telephone / Cellphone number(s) of the claimant (Assured & Third Party).

Motor Claim Requirements

Own Damage (Insured Vehicle).
1. Photocopy of Insurance Policy with Official Receipt (insured vehicle) and Endorsement, if any.
2. Original Police Report / Barangay Incident Report.
3. Photocopy of Driver’s License with Official Receipt.
4. Photocopy of Registration Certificate of the insured vehicle with Official Receipt.
5. Pictures of damaged portion and full body of the vehicle showing plate number ( at least 4 angles).
6. Repair Estimate of our accredited shops ( at least 2 ).
7. Stencils of motor and serial numbers.

Own Damage with Third Party Property Damage.
1. Photocopy of Insurance Policy with Official Receipt (insured vehicle).
2. Original Police Report / Barangay Incident Report.
3. Photocopy of Insurance Policy with Official Receipt (insured vehicle).
4. Affidavit / Statement of both driver’s involved in the incident.
5. Photocopy of Driver’s License with Official Receipt (both parties).
6. Photocopy of Registration Certificate with Official Receipt (both parties).
7. Pictures of damaged portion and full body of the vehicle showing plate number ( at least 4 angles of both parties vehicle).
8. Repair Estimate of our accredited shops ( at least 2 of both parties vehicle ).
9. Stencils of motor and serial numbers (both parties).
10. Certificate of No Claim from the insurer of the adverse party.

Third Party Property Damage
1. Photocopy of Insurance Policy with Official Receipt (insured vehicle).
2. Original Police Report / Barangay Incident Report.
3. Affidavit / Statement of both driver’s involved in the incident.
4. Original Certificate of No Claim from the insurer of the adverse party.
5. Photocopy of Driver’s License with Official Receipt (both parties).
6. Photocopy of Registration Certificate with Official Receipt (both parties).
7. Pictures of damaged portion and full body of the vehicle showing plate number ( at least 4 angles).
8. Repair Estimate of our accredited shops (at least 2).
9. Stencils of motor and serial numbers (both parties).

CARNAP
1. Photocopy of Insurance Policy with Official Receipt (insured vehicle).
2. Original Affidavit / Statement of the driver.
3. Photocopy of Driver’s License with Official Receipt.
4. Photocopy of Registration Certificate of the insured vehicle with Official Receipt.
5. Original Complaints Sheet.
6. Original Alarm Sheet.
7. Original Certificate of Non-Recovery for 90 days.
8. Original Certificate of Registration & Official Receipt of the insured vehicle (if not recovered within 90 days).
9. Key ignition of the insured vehicle (if not recovered within 90 days).



What to do in case of claim?

1. Seek the assistance of Traffic Officer.
2. DO NOT ADMIT LIABILITY.
3. Take photos of the accident scene (preferably wide-angle shots showing plate number, damages sustained by the vehicles involved with land mark of the place of accident).
4. Secure the following information or (clear photos of):
1. Third Party’s vehicle plate number/make/model/color
2. Driver’s License and Contact Numbers
3. Adverse Vehicle’s Certificate of Registration and its Official Receipts
4. Insurance Policy details
5. Identification of the attending Traffic Officer (Name, rank and Precinct or Organization he/she belongs)
5. Assist injured parties. If in danger, immediately report the incident to the nearest police station.
